1) Hong Kong Identity Card
2) Proof of present address which is issued not more than 3 months
3) Bank account for payroll (such as bank book or debit card)
4) Original copy of related nursing certificate (Practicing Certificate should be provided by registered nurses and enrolled nurses)
5) One piece of passport photo
